
Welcome To PATCH
From 2006-2010, the PATCH program provided geriatric palliative home care for elderly who were nearing the end of life and suffering with multiple, chronic medical problems. PATCH palliative care services sought to optimize quality of life through services addressing medical and psychosocial needs and varying from longitudinal care to sub-acute care and consultation. PATCH also trained medical students, residents and fellows at the University of Chicago through home visits, lectures, independent study modules and online learning activities. Although PATCH services were discontinued in the summer of 2010, the curriculum remains online as a resource for other trainees.
